Middle Cerebral Artery Bifurcation Aneurysm: Acute Subarachnoid Hemorrhage Secondary to a Saccular Aneurysm Arising from the Bifurcation of the Middle Cerebral Artery – Endovascular Treatment with the Woven EndoBridge Device

Abstract

A 39-year-old male was admitted with a sudden onset right-sided headache and presented to the emergency department 2 h after ictus. The patient had a GCS of 15 with no localizing neurology. A CT head scan showed a right-sided frontoparietal hematoma with an associated subarachnoid hemorrhage (SAH) WFNS grade 1/Fisher grade 4. An intracranial CT angiogram showed a 10 mm saccular aneurysm at the bifurcation of the right middle cerebral artery. The patient underwent a DSA, which confirmed the location of the aneurysm. A Woven EndoBridge (WEB) device was used to exclude the aneurysm from the cerebral circulation. Post-deployment angiographic images confirmed correct placement across the aneurysm neck. The patient was discharged 2 weeks later at his neurological baseline. The current evidence for the use of the WEB device in ruptured intracranial aneurysms, its effectiveness, and potential complications are the main topics of this chapter.
