Abstract

Lung transplantation is the only effective treatment for children with end-stage lung disease or pulmonary vascular disease, in which all medical therapies have been exhausted. Cystic fibrosis is the most common indication. Pulmonary hypertension has become a much less common indication, as medical therapy for this has improved substantially over the past decade. An emerging indication is congenital abnormalities in surfactant production and/or metabolism. Although there has been some improvement in outcomes since the onset of lung transplantation, 5-year mortality still hovers around 50%. The limiting factors in successful outcome are donor availability, early graft failure, infection, and bronchiolitis obliterans.
