Condyloma Acuminatum
Definition
They are benign, flat, or verruciform lesions caused by low-risk HPV.
Clinical Features
They may be asymptomatic and incidental. If at the introitus, they may present with itching and irritation.
Incidence
They are rare in the vagina.
Age
Generally seen in young, sexually active adults.
Site
Lower vagina and introitus.
Treatment
Nonsurgical treatment including observation, local excision, topical imiquimod, and laser ablation.
Outcome
Multiple recurrences are not common (Widschwendter et al. 2019).
Macroscopy
Hyperkeratotic flat or raised lesions.
Microscopy
Acanthosis, blunt papillomatosis, tiered parakeratosis, hypergranulosis, hyperkeratosis, and koilocytosis can be seen. The prominence of the features correlates with the age of the lesion, being more prominent in younger lesions.
Immunophenotype
P16 staining is nonblock. Low-risk HPV (usually HPV6 and HPV11) can be detected by PCR or in situ hybridization (Pirog 2015).
