Abstract
A partially thrombosed dissecting fusiform aneurysm of the left posterior cerebral artery (PCA) was found in a 55-year-old male patient, who presented to hospital with slurred speech and a right hemiparesis. The patient underwent elective treatment of the aneurysm with placement of a p64 flow diverter stent (FDS). A follow-up digital subtraction angiography examination at 6 months demonstrated the patency of the left PCA as well as complete vascular remodeling of the fusiform aneurysm along the p64 FDS. The patient demonstrated clinical improvement with moderate residual cognitive impairment and mild residual right-sided weakness. Flow diversion may be a viable approach for patients presenting with such aneurysms, when the aneurysm characteristics preclude either surgical bypass or parent vessel sacrifice. The reconstructive treatment of dissecting aneurysms of the PCA is the main topic of this chapter.
