Abstract
Pediatric solid-organ transplantation is a maturing specialty that has evolved from the close collaboration between surgical and immunological research. It continues to challenge practitioners, both technically and in understanding the underlying processes that allow the human body to distinguish self from foreign tissues. A multidisciplinary approach to both assessment and management of these complex patients is essential for long-term success. Constant innovation in immune suppression, surgical techniques, and organ procurement has advanced pediatric transplantation survival in a relatively short period of time but is still far from attaining the ultimate goal of graft tolerance. The indications for transplantation are ever increasing, placing demands on organ supply which needs to be managed by appropriate legislation. Different countries have taken different approaches to this legislation and are heavily influenced by both cultural and ethical factors.
