Zusammenfassung
Epilepsien im Kindesalter unterscheiden sich von denjenigen im Erwachsenenalter durch eine Reihe von Faktoren. Die Inzidenz von Epilepsien und epileptischen Anfällen ist im Neugeborenen-, Säuglings- und Kindesalter besonders hoch, die Diagnostik ist durch die Unreife des kindlichen Gehirns und der damit verbundenen untypischen und oft subtilen Anfallsmorphologie erschwert. Die Möglichkeiten zur medikamentösen Behandlung sind begrenzt, da für viele antikonvulsiv wirksame Substanzen gerade im frühen Kindesalter die Studienlage bisher keine ausreichende Evidenz besitzt. Dabei ist eine exakte Diagnose und frühzeitige Behandlung von besonderer prognostischer Bedeutung. Beispiele dafür sind die epileptischen Enzephalopathien, die häufig im frühen Kindesalter manifest werden. Bei epileptischen Enzephalopathien führen die Anfälle zu einer deutlich schwereren kognitiven Beeinträchtigung, als diese durch die zugrunde liegende Schädigung (und Anfallsursache) zu erwarten wäre. Neugeborenenanfälle oder solche, die im frühen Kindesalter auftreten, sind häufig auch Symptom anders zu behandelnder Zustände (Elektrolytstörungen) oder Stoffwechselerkrankungen. Auch hier ist eine exakte Analyse der Anfallsursache von besonderer Bedeutung. Die sog. elektroklinischen Syndrome bezeichnen epileptische Syndrome, die durch charakteristische EEG-Muster und typische Klinik zuverlässig einzuordnen sind. Sie entstehen ebenfalls überwiegend im Kindes- und Jugendalter. Die Internationale Liga gegen Epilepsie (ILAE) hat kürzlich die neue Klassifikation der epileptischen Anfälle sowie die der Epilepsien und epileptischen Syndrome vorgestellt. Sie enthalten eine Vielzahl von Änderungen in der bisherigen Einordnung von Anfällen und Epilepsien der bisher gültigen Klassifikationen der ILAE. Auch werden viele bisher gebräuchliche Begriffe nicht mehr empfohlen. Im bisherigen Sprachgebrauch haben sich gerade für die Beschreibung von Syndromen Namen etabliert, zu deren künftigen Verwendung in den bisher zur Verfügung stehenden Publikationen nicht explizit Stellung genommen wird. In der folgenden Darstellung der Epilepsien des Kindes- und Jugendalters werden die epileptischen Anfälle entsprechend der neuen Nomenklatur benannt. Die bisher üblichen Bezeichnungen der speziellen Syndrome werden beibehalten.
