Zusammenfassung
Zur Thrombenbildung im Herzen kommt es bei pathologischen morphologischen Veränderungen oder Rhythmusstörungen. Nach einem Myokardinfarkt kann durch die Ausbildung eines hypokinetischen Wandareals in etwa 2,5 % der Fälle in den ersten 6 Wochen eine zerebrale Ischämie auftreten. Im chronischen Stadium stellt die Ausbildung eines Herzwandaneurysmas eine mögliche Emboliequelle dar. Selten ist das gutartige Vorhofmyxom, das sich typischerweise durch einen embolischen Hirninfarkt manifestiert. Bei Kindern und Jugendlichen spielen angeborene Herzvitien eine Rolle als Emboliequelle. Entzündliche Erkrankungen der Herzklappen sind ebenfalls mit embolischen Ereignissen assoziiert, wobei in der Akutphase besonders septische Embolien das Krankheitsbild komplizieren. Chronische Mitral- oder Aortenklappenprozesse gehen nicht selten mit Vorhofflimmern einher, was die Thrombenbildung im linken Vorhof begünstigt.
