Zusammenfassung

Thrombosen der zerebralen Sinus und Venen sind eine klinische Entität, deren tatsächliche Inzidenz aufgrund ihrer extrem heterogenen Symptomatik unbekannt ist. Zudem sind Kopfschmerzen nicht selten alleiniges Symptom, sodass wahrscheinlich viele (oligosymptomatische) Fälle unerkannt bleiben. Auch bei Patientinnen mit den typischen Symptomen eines Pseudotumor cerebri muss an eine zerebrale Sinusthrombose gedacht werden. Sowohl die CT-gestützte Venografie als auch die MR-Venografie sowie blutsensitive MR-Sequenzen sind zum Nachweis bzw. Ausschluss intrakranieller Venenthrombosen geeignet, wohingegen, anders als bei Thrombosen von Bein- oder Beckenvenen, Laborwerte (D-Dimere) keine ausreichende diagnostische Sicherheit bieten. Trotz geringer Evidenz gilt die Antikoagulation sowohl in der Akutphase als auch in der Sekundärprävention als Therapie der Wahl.
