Zusammenfassung
Die Blutung aus den großen Gefäßen des Circulus arteriosus Willisii führt aufgrund der anatomischen Gegebenheiten zu einer Subarachnoidalblutung (SAB) mit dem Leitsymptom heftigster Kopfschmerz. Die wichtigste Ursache sind zerebrale Aneurysmen (85 %). Darüber hinaus können Verletzungen, andere vaskuläre Malformationen, Neoplasmen, Gerinnungsstörungen, ein reversibles zerebrales Vasokonstriktionssyndrom (RCVS), Medikamentenabusus oder entzündliche Gefäßerkrankungen zugrunde liegen. Mit einer Mortalität von 25 % und einer Morbidität von 50 % der Überlebenden ist die SAB ein gefährliches Krankheitsbild.
