Zusammenfassung

Das menschliche Hämostasesystem befindet sich physiologischerweise in einem natürlichen Gleichgewicht zwischen pro- und antithrombotischen Prozessen, um einerseits im Falle einer Verletzung durch eine schnelle Gerinnselbildung ein Verbluten zu verhindern, andererseits aber auch den Blutfluss aufrechtzuerhalten. Dies wird mit genau gesteuerten Regelkreisen, die die hämostasefördernden und die gegenläufigen hämostasehemmenden Prozesse in einem fein austarierten Gleichgewicht halten, erreicht. Ein Ungleichgewicht in diesem komplexen System führt auf der einen Seite zu hämorrhagischen Diathesen mit dem Risiko von Blutungen und auf der anderen Seite zu thrombophilen Diathesen mit dem Risiko von Thrombosen und Embolien. Beide Situationen können hereditär, genetisch determiniert, oder sekundär, erworben, auftreten. Die korrekte Diagnosestellung ist aufgrund der hohen klinischen Varianz der Krankheitsbilder komplex. Die klinische Forschung identifiziert zudem immer mehr Ursachen von hämostaseologischen Störungen. Demgegenüber stehen aber auch stetige Neuentwicklungen von medikamentösen Therapien für alle Bereiche der Hämostaseologie, insbesondere auf dem Gebiet der Antikoagulanzien und Orphan Drugs.
