Zusammenfassung
Bei afferenten Störungen des Pupillenreflexes fehlen bei Belichtung der betroffenen Pupille direkte und konsensuelle Lichtreaktion, während bei Belichtung des kontralateralen Auges die Antwort auf beiden Seiten normal ist (afferente oder amaurotische Pupillenstarre). Ein relativer afferenter Pupillendefekt kann im Swinging-flashlight-Test detektiert werden. Bei einer Okulomotoriusparese kommt es zu einer absoluten Pupillenstarre. Eine Light-Near-Dissoziation liegt bei der Pupillotonie und der Argyll-Robertson-Pupille vor. Bei der Retrobulbärneuritis oder der Kompression des N. opticus kommt es zu einer Rot-Grün-Schwäche mit Visusstörung. Monokuläre Gesichtsfelddefekte sind auf Schädigungen der Retina oder des N. opticus zurückzuführen, binokuläre Läsionen können Folge bilateraler Optikopathien oder einer Schädigung des Chiasmas (heteronyme Ausfälle) oder der retrochiasmalen Sehbahnen (homonyme Ausfälle) sein.
