Zusammenfassung
Schwindel ist mit 30–39 % Lebenszeitprävalenz eines der häufigsten Symptome in der klinischen Neurologie, in einer interdisziplinären Notaufnahme ist Schwindel in ca. 5 % das führende Leitsymptom. Es handelt sich nicht um eine homogene Krankheitsentität, sondern um ganz unterschiedliche Syndrome, denen eine gestörte Interaktion (Mismatch) der an der dynamischen Raumorientierung beteiligten multisensorischen Sinnessysteme (visuell, vestibulär, propriozeptiv) und deren Abgleich mit der Motorik zugrunde liegt. Dieses komplexe Regulationssystem muss zur Aufrechterhaltung einer stabilen Raumwahrnehmung durch Bewegung ständig neu geeicht werden. Das setzt eine erhaltene Plastizität der daran beteiligten neuronalen Prozesse voraus und ist nach Läsionen im vestibulären System, im höheren Alter und bei psychischem Stress besonders leicht störbar. In etwa 50 % liegen vestibuläre Ursachen vor. Aufgrund ihres klinischen Bildes kann man sie meist durch sorgfältige Anamnese und Untersuchung von nichtvestibulären Schwindelursachen differenzieren. In diesem Kapitel werden Wege zu einer effizienten Differenzialdiagnose und Therapie der wichtigsten Schwindelsyndrome aufgezeigt.
