Zusammenfassung

Monoklonale Immunglobuline können zu einer Schädigung des peripheren Nervensystems führen. Bei einer monoklonalen Gammopathie unbestimmter Signifikanz (MGUS) müssen stets maligne Prozesse (multiples Myelom, Plasmozytom, Makroglobulinämie Waldenström, Amyloidose) ausgeschlossen werden! Im Rahmen neoplastischer Erkrankungen können periphere Nerven durch Kompression oder Infiltration, Malnutrition (Vitamin-B12-Mangel) und toxische Ursachen (Vincristin, Cisplatin) geschädigt werden. Wenn Neuropathien durch Fernwirkung (Remote-Effekt) der Tumoren entstehen, spricht man von paraneoplastischer Polyneuropathie. Bei etwa 1 % der Krebspatienten ist eine klinisch manifeste Polyneuropathie nachweisbar. Am häufigsten sind sie beim Mamma- und beim Bronchialkarzinom sowie bei Lymphomen.
