Zusammenfassung
Der Hirnabszess ist eine Form der eitrigen Reaktion mit Gewebseinschmelzung und Kapselbildung, die in der Mehrzahl der Fälle hämatogen-metastatisch oder fortgeleitet, am häufigsten bei Otitis, Sinusitis und Mastoiditis, entsteht. Nur 10 % der Hirnabszesse werden durch offene Schädel-Hirn-Traumen oder neurochirurgische Operationen hervorgerufen. Die Therapie erfolgt antibiotisch und ggf. neurochirurgisch. Eine weitere fokale intrakranielle Infektion ist das Subduralempyem, ein eitriger Erguss in dem von harten und weichen Hirnhäuten präformierten Hohlraum. Das als „dringlichster aller neurochirurgischen Notfälle“ beschriebene Subduralempyem macht 10–20 % der fokalen intrakraniellen bakteriellen Infektionen aus.
