Abstract

Advance care planning is a person-centered process to enable a person to express their preferences for future health care. Advance care planning is an integral component of palliative care for people at various stages in the illness trajectory. This chapter provides an overview of advance care planning; the potential benefits, barriers, and facilitators; the key components of advance care planning at various stages of health; strategies for the advance care planning conversation; and areas for future research.
